Support for Health Partners
In response to changes in health care, many hospitals, physicians’ groups and Accountable Care Organizations are looking to partner with community-based service providers like Springwell to help coordinate medical care with home-based services options in order to improve long-term health outcomes and decrease health care costs.
Community Linkages services offered through Springwell’s Health Partnership Program offers Accountable Care Organizations, hospitals, and insurers an array of tailored services that support the triple aim goals of improving patient experience and health and reducing the cost of health care.
Springwell has experience collaborating in health care settings combined with more than 30 years of coordinating social services. We understand the interdependence of health and social issues. Our Health Partnership Program provides Care Linkages support that helps ensure patients receive the community supports necessary to meet health care goals. Springwell staff are experienced in focusing on patients at risk of non-engagement, hospitalization, or who are difficult to reach. We can help improve communication at transition points between home, physician, hospital, and rehab. Springwell staff can augment RN and Social Work staff efforts with focused telephonic or in-home, in-hospital and in-rehab visits that enhance and strengthen efforts of in-house staff. Tailored services are focused on ensuring that patients have the environmental and long-term supports that are vital to their acute and chronic health care needs.
Springwell has worked with a number of different health care providers to pilot this program.
