CareConnections®

You don't need to move to an Assisted Living facility to get the support you need.Springwell's CareConnections program brings support to people right where they live, in surroundings they are familiar with. Through CareConnections, residents of senior housing have preferred access to a variety of social, in home and health care services at reduced rates.

CareConnections offers housing authorities and private management companies a menu of comprehensive supportive services that helps assure the safety and independence of the residents, including:

  • A Care Coordinator on-site for up to twenty hours each week, who
  • works closely with residents and family to assess needs and eligibility for subsidized programs and who coordinates and monitors service delivery;
  • Regular Care Coordinator office hours to meet with residents or their family members;
  • Coordinated personal care & homemaking services from a single, highly qualified service provider;
  • Personal Care / Homemaking services available in 15-minute increments;
  • An on-site Personal Care Homemaker to respond to urgent needs during evenings, overnight, weekends, and holidays;
  • On-site meal program;
  • Social activities that reduce isolation, develop community, and monitor frailer residents in an informal setting;
  • Unlimited access to Springwell's professional staff 24-hrs, 7 days a week for non-medical urgent situations; and
  • Weekly on-site nurse monitoring of blood pressure and pre-filling of medication.

The COMMONS at Nonantum
239 Watertown Street , Newton, MA

Located in the center of vibrant Nonantum Village, The Commons at Nonantum Village offers a unique group living option for seniors. Residents enjoy the privacy of their own apartment and half bathroom and are part of a community that shares a living room, dining room and kitchen. Care management and supportive services are available including assistance with:

  • Bathing
  • Dressing
  • Housework
  • Laundry
  • Meals
  • Medication
  • Food Shopping
  • Medical Transportation

ENHANCED CARE

For those needing a higher level of support, Springwell and the Newton Housing Authority are proud to offer an Enhanced Service option at The Commons. Daily help with tasks that help each resident to maintain or enhance their independence is available.  In addition, a professional staff person is on site from 7:00 p.m. through 7:00 a.m. providing peace of mind and an increased sense of security knowing that assistance is available should it be needed.

ELIGIBILITY

Individuals must be 60 years of age or older and must meet financial eligibility guidelines as established by the Newton Housing Authority. 

A free consultation with you and your family can help you decide if The Commons at Nonantum Village is right for you. A Springwell Care Coordinator will visit with you at your home.  If you are living in a nursing home or other facility we will visit you there. We know that every resident and their needs are unique.  Our Care Coordinator will work closely with you to understand your needs and preferences.

Congregate Housing Program

Congregate Housing

is a unique group living option for elders. Residents have a private area which they furnish themselves. Congregate residents also have access to numerous common areas, including kitchen, living room, and dining room. This design provides residents with both privacy and the opportunity for companionship.

Supportive Services

are available to residents on an as-needed basis. Maintaining residents’ dignity and an independent lifestyle is a major goal of the Congregate Housing Program.

What services are available?

Support service plans are determined by the Congregate Housing Coordinator based on individual need. Some of the options available are:


  • Information and Referral
  • Personal Care
  • Housekeeping
  • Care Management
  • Medical Transportation
  • Laundry
  • Food Shopping
  • Meal Coordination

People who live in congregate housing are:

  • 60 years of age and older, whose income falls within financial guidelines defined by the local housing authority
  • Able to live independently with the supportive services available
  • Able to transfer, dress, toilet, and feed themselves with supportive services if needed
  • Able to manage their own medications with supportive services if needed
  • Emotionally stable
  • Interested in living cooperatively with others
  • Willing to commit to remaining in congregate housing for a minimum of one year.
